APPLICATION INSTRUCTIONS – Please review carefully
1. This application must fall into one of United Way of Greenbrier Valleys’s four Focus Areas: Health, Financial Stability, Education or Basic Needs. The application should present a project(s) that addresses at least one Focus Area and presents measurable goals.
2. Agencies with annual gross revenue of $500,000 or less will not be required to submit an IRS Form 990. All others are asked to email a copy of their most current IRS Form 990 or IRS Form 990-EZ to firstname.lastname@example.org. EXCEPTION: All first time applicants MUST file their most current 990-N, 990-EZ, or 990.
3. In accordance with W. Va. Code § 29-19-5(a)(6)(A)-(B):
a. Charitable organizations with annual contributions, not including grants from government agencies or private foundations, of over $500,000 must file, upon request, an audited financial statement prepared by an independent CPA with their grant application.
b. Charitable organizations with annual contributions in excess of $300,000 but not more than $500,000 must file, upon request, a financial statement reviewed by an independent CPA with their grant application.
c. Charitable organizations with annual contributions less than $300,000 are NOT required to submit any financial statement from an independent CPA with their grant application.
